Loan Modification Checklist

The following checklist represents items to include in a complete package for your lender if you are applying for loan modification. Some items may be supplied by the lender or attorney handling your case. You should make every effort to provide all items to increase your chance for a successful modification.

  • Cover letter, stating purpose and contents of package
  • Client Authorization Form (signed by all borrowers, one for each lender)
  • IRS Form 4506-T, Request for Transcript of Tax Return (one for each borrower)
  • Two months’ pay stubs for all borrowers
  • Year-to-date Profit & Loss statements for each business (self-employed and entrepreneur borrowers only)
  • Hardship letter and any documentation to back up claim
  • Schedule of real estate owned, especially if more than one property
  • 1003 Loan Application
  • Recent appraisal or county property value assessment
  • Recent real estate tax bill, especially if taxes are not part of mortgage payment
  • Proof of Homeowner’s Insurance
  • Copy of personal budget for all borrowers (lender will usually supply)
  • Recent copy of credit report, if available
  • Two most recent statements for all mortgages
  • Two most recent bank statements
  • Letters from credit counselors, if applicable
